Facilitate critical healthcare transitions for military personnel and their families at a premier medical facility in Germany. Coordinate referrals, manage records, and ensure smooth patient handovers between departments.

As a Clinical Transition Representative (OCONUS), you will serve as the primary liaison between the healthcare facility, patients, and military command to ensure seamless transitions for service members and their families. You will coordinate medical referrals, manage patient records, and facilitate communication across departments in a fast-paced, tri-service environment. This role requires a deep understanding of clinical processes, excellent organizational skills, and the ability to work independently while supporting the mission of world-class patient care.

Key Responsibilities:

• Coordinate and schedule patient transfers between military hospitals, clinics, and civilian healthcare providers.
• Act as the point of contact for all clinical transition requests, ensuring compliance with regulatory and command directives.
• Maintain accurate electronic health records (EHR) and update transition tracking systems.
• Collaborate with case managers, social workers, and medical staff to develop individualized care plans.
• Resolve logistical issues related to transportation, documentation, and cross-border medical authorizations.
• Provide briefings to command leadership on transition metrics and process improvements.
• Ensure patient confidentiality and adherence to HIPAA and local data protection laws.

• Education: Bachelor's degree in Healthcare Administration, Nursing, or related field; equivalent experience considered.
• Experience: Minimum 2 years in a clinical coordination, patient advocacy, or military medical administration role.
• Certification: Must obtain or already hold the Customer Relations Certification (provided upon hire).
• Skills:
  • Proficiency in Microsoft Office Suite and EHR systems (e.g., MHS GENESIS).
  • Strong verbal and written communication in English (German language skills preferred).
  • Ability to prioritize tasks in a high-volume environment.
  • Knowledge of TRICARE, NATO medical protocols, or overseas healthcare logistics.
• Clearance: Must be eligible for a NATO Secret security clearance or possess a U.S. Secret clearance.
• Other: Willingness to work occasional weekends or holidays during peak transition periods.
